Specifically, PHI is information about you, including demographic information (i.e., name, address, phone, etc.), that may identify you and relates to your past, present or future physical or mental health condition and related healthcare services.
- AllergiesDr. Dubravec attended college and medical school at the University of Illinois, followed by residency at Madigan Army Medical Center in Tacoma, Washington. He completed Allergy-Immunology (adult and pediatric) training at Fitzsimons Army Medical Center in the Denver area (affiliated with National Jewish Center), and went on staff at Brooke Army Medical Center (the Army's teaching hospital in San Antonio, TX) where he became Chief of Allergy & Immunology and served as affiliated staff at the Air Force's Allergy-Immunology training program at Wilford Hall Medical Center. After finishing his military obligation in 1999, he practiced in Kalamazoo, MI for 1 year, followed by 2 years in the Salt Lake City area while his wife finished her training. In 2002 he then returned to Michigan with his family to settle in the Grand Rapids Area and start his private Allergy practice.
